Several of these frequently coexist, which is the usual reason a man who was treated for one of them is still tired.

Where testosterone actually sits in this

Low testosterone can cause fatigue, but it is nowhere near the most common cause, and the evidence on treating fatigue with testosterone is not encouraging.

In the Testosterone Trials, vitality and energy scores did not improve significantly in men treated for a year, even though libido, anaemia and bone density did. Mood improved slightly.

So a man whose only symptom is tiredness is unlikely to get what he wants from testosterone, even if his level is at the lower end.

The sleep point that decides most cases

Untreated obstructive sleep apnoea causes exactly this picture, and it also lowers testosterone directly. Treating the apnoea often resolves both, and treating the testosterone while ignoring the apnoea resolves neither.

Snoring, witnessed breathing pauses and waking unrefreshed are asked about at every one of these consultations for that reason.

Possibly, but it is one of the less likely explanations. Sleep apnoea, short sleep, iron deficiency, thyroid disease, poorly controlled glucose, depression and medication are all more common causes of the same picture.

It depends what was actually tested. Ferritin, thyroid, vitamin D and HbA1c are often missing from a basic panel, and a morning fasting testosterone is often not done correctly. A normal narrow panel is not a complete work up.

Yes. Low ferritin causes fatigue, poor recovery and reduced training capacity well before haemoglobin falls, and it is one of the most commonly missed findings in this group.

It can, and it is asked about directly. The distinguishing feature is usually loss of interest and pleasure rather than energy alone. Where that is the picture, the right treatment is not a hormone.

It depends on the cause. Sleep changes show within weeks, iron and vitamin D take weeks to months, and glucose and weight take longer. Blood work is repeated at around three months.

Frequently. Beta blockers, some antidepressants, sedating antihistamines, long term opioids and high dose steroids all contribute, and the last two also suppress testosterone directly.

That is the usual situation, and it is why treatment is sequenced. Fixing one factor and ignoring the rest is the most common reason men stay tired after seeing a doctor.
