Why sleep gets chased first

Chronic short sleep and untreated apnoea produce exactly this presentation, and they are both common in men with demanding jobs in Dubai.

Treating everything else while sleeping five hours a night produces very little. Treating the sleep frequently resolves the fog on its own.

Where testosterone sits

Low, in terms of likelihood. In the Testosterone Trials, a subgroup of 493 men with age associated memory impairment showed no improvement in memory, executive function, visual memory or spatial ability after 6 and 12 months of testosterone.

So testosterone is not a treatment for brain fog. Where deficiency is genuine and there are other symptoms, treating it is reasonable, but expecting cognition to improve is not supported.

What should worry you

Brain fog is not dementia, and the distinction is usually clear.

  • Fog affects concentration and word finding. Getting lost in familiar places, forgetting whole recent events or difficulty with everyday tasks is different and needs a different assessment.
  • Sudden onset, or fog with weakness, speech difficulty or visual change, needs urgent assessment rather than a work up.

Full blood count and ferritin, vitamin B12 and vitamin D, thyroid function, HbA1c and glucose, and kidney and liver function. Those four findings explain a large share of cases.

Because deficiency causes exactly this and is common in men on long term metformin or acid suppressing medication, and in those eating little animal food. It is easily corrected once found.
